Wrong Drug/Wrong Dose
One of the most common medical errors overall is the administration of the wrong drug or the wrong dose, and these errors happen all of the time in the hospital. Actor Dennis Quaid was in the news a few years ago because his infant twins were administered a blood thinner about 1,000 times stronger than what the doctor prescribed. This created a life-and-death crisis which the babies survived. Quaid reported to 60 Minutes the startling fact that his children got the wrong drug administered twice. This particular medication error is nothing new. It even had happened before at that same hospital, resulting in the preventable death of an infant.
