Wrong Site Surgery
The government has said that “wrong-site surgery” should be a “never event,” that is, something that simply does not happen in American hospitals. It’s far from a never event – it happened 867 times in 2009 but that reported number is likely much too low. The reason why – hospitals don’t have to report this kind of error! And up until 2009, Medicare would still reimburse the hospital even if it amputated the wrong leg.
